First guess is intercooler condensation. Is it throwing the wrench icon on the dash ?

The crazy part is that it feels like something is holding the truck back the. Like something gives them the boost kicks in and the truck takes off bu then It starts to sputter

With P-codes & wrench on dash, what you are describing isn't crazy at all.

Vehicle is having drivability issues and putting itself in limp mode.

I think Arkansas Eco is probably right, sorry to say.
